There is a service member who would like to disenroll from the EFMP. What guidance do I provide for a service member who would like to disenroll from the EFMP?

There are four qualifying factors that would warrant a dis-enrollment from the EFMP. Please review the below information and complete what is required in order to begin the dis-enrollment process. Example: EFM no longer has speech therapy. Dis-enrollment guidance for option #1 would be completed in order to dis-enroll the EFM from the EFMP.  Action is required to dis-enroll from the EFMP. Failure to update EFM’s enrollment or electing not to update enrollment does not equate to a dis-enrolled case status and could possibly hinder the service member’s ability to negotiate future orders. 

 

  1. Resolution of Needs (no longer having a qualifying condition): Submit an updated DD 2792 (medical summary) for medical and psychological enrollments or submit a DD 2792 (medical summary), DD 2792-1 (educational summary), and closed out IEP ​for special education enrollments 
  2. Change in Family Status (divorce, loss of custody etc.):  Submit a serialized dis-enrollment letter signed by the CO or whomever has signature authority for the CO. A sample dis-enrollment letter is attached to this email to be utilized as a template. 
  3. Death of the EFM: Submit a serialized dis-enrollment letter signed by the CO or whomever has signature authority for the CO. A sample dis-enrollment letter is attached to this email to be utilized as a template. 
  4. Separation from the Service/Retirement: Answer the following questions and email them to your assigned case liaison so that he/she may be able to request that the case be dis-enrolled. The case will be dis-enrolled the day after your retirement date. For example, date of retirement is 01 SEP 2020. The earliest your assigned case liaison can request the case to be dis-enrolled is 02 SEP 2020.
    1. ​Have you formally requested retirement?
    2. Have you received message traffic indicating provisional approval of your requested date?
    3. What is your actual retirement date?
    4. When will you begin terminal leave?
