To obtain a New ID Card for an Illegitimate Child or Child out of Wedlock:

1. A male sponsor must provide the following:

• Court order that establishes paternity or an approved dependency determination of a notarized voluntary acknowledgement of paternity
• Child’s original or certified copy of the birth certificate
• Original Social security card (if the child is over 2 months old)

2. A female sponsor must provide the following:

• Child’s original or certified copy of the birth certificate
• Social security card, if available

Note: All voluntary acknowledgements of paternity must be notarized and original to have the child added to DEERS. Voluntary acknowledgement of paternity must be completed in the state where the mother and child reside. This applies to Army Soldiers; requirements differ for other branches of service.
