**Note** The DFAS ID Card Office is currently offering appointments for  customers that reside over an hour away from site location and walk-ins for local residents.  Walk-ins will be called by tickets issued.  To make an appointment call (317) 212-4430.  DFAS employees need to bring current CAC, Driver's License, and Laptop.

Promotion or Demotion

Here are the necessary procedures to follow to obtain a new identification card due to promotion/demotion:

  • Promotion: New ID cards are issued to Service Members promoted to E-4 and above. Required documents include promotion orders, old ID card and another valid form of identification. Social Security Card or Original Birth Certificate.
  • Demotion: Required documents include DA Form 4187, reduction orders, or a memorandum showing new rank and effective date.
