Name Change

Active/Reserve/National Guard Service Members must provide an approved memorandum or DA Form 4187 requesting your name change; this must be done through your S-1 and/or Unit Admin (UA).

Spouses/Family Members must have 2 valid State or Federal IDs with the legal name that they want to be enrolled with. This also applies to civilian employees who marry or divorce and need to have their name changed.

DoD Civilians/Contractors needing name change must go through their HR and/or Trusted Associate Sponsorship System (TASS) representative to ensure the appropriate change is fed to DEERS/RAPIDS.

Note: The primary State or Federal ID (i.e. Driver's License) used cannot be expired.
