Replace a Lost or Stolen ID Card

DoD Civilians

DoD Civilians need 3 items:

  1. Memo from your supervisor stating CAC is lost and needs replacement.
  2. Two forms of identification such as: Driver's license (cannot be expired), Social Security Card, or original Birth Certificate.

DoD - DFAS Employees and Contractors (ONLY) need 3 items:

  1. Two forms of identification such as: Driver's license (cannot be expired), Social Security Card, or original Birth Certificate.
  2. Have supervisor present

Military (All Branches)

For stolen CAC cards, bring:

  • A police report stating that CAC was stolen.
  • Two forms of identification such as: Driver's license (cannot be expired), Social Security Card, or original Birth Certificate.

For lost CAC cards, bring:

  • A Counseling Statement or Unit Letter from your Unit or Commander reporting the CAC was lost.
  • Two forms of identification such as: Driver's license (cannot be expired), Social Security Card, or original Birth Certificate.

Family Members and Retired Service Members

Family Members need 4 items:

  1. Two forms of identification such as: Driver's license (cannot be expired), Social Security Card, or original Birth Certificate.
  2. One of the following:
    • Have sponsor present OR
    • General/Specific Power of Attorney (specifying DEERS/ID Cards) OR
    • Original DD Form 1172-2 signed by the sponsor and ID Card Issuing Facility verifying officer
